Please follow these guidelines to assure the safety and comfort of anyone who enters.
- Please arrive at the time of your scheduled visit.
- Please use hand sanitizer or wash hands at sink upon entering.
- Remain at all times, when possible, 6 feet in distance from anyone else in the space.
- Please bring personal items, clothing, etc. into the treatment room with you. Shoes will remain in the reception area under a chair or on the shoe rack. No food or drink (besides covered water container) should be consumed in office, please plan accordingly.
- If you have ANY symptoms of COVID and/or cold-virus, please postpone your visit until feeling 100% symptom-free. Symptoms include (not limited to): fever, runny nose, cough, sore throat, severe headache, extreme and unusual fatigue, difficulty or shortness of breath.
- Before your Appointment: Advise your provider of any COVID exposures within the prior week of your scheduled visit.
- In the week following your office visit, if you should receive a positive COVID test, you will contact your provider immediately.
- Provider has a right to end the session and refuse future visits if the above guidelines are not met.
